Parents and Players,
Please be advised that by state law we are now required to collect and maintain a signed Concussion form from each player and parent. These forms are available at the link on the right top of this page. Please print and sign your form and bring it with you to the games this Sunday. We must have a record of receiving this form before your player will be allowed to play.
